Medical Records

Your medical records contain your personal health history, which supports continuity of care. Records are private and secure, and are never released without your written consent, unless required by state or federal law (e.g., subpoena, court order, public health reporting, or Notice of Privacy Practices).

SHS retains medical records for at least six years after your last visit to the Medical Clinic.

Requesting Copies of Your Medical Record

Copies of your SHS medical record are available with a signed consent for release (must be signed manually).

Submit the consent form:

  • In person at the Student Services Building, Room 203
  • Electronically 
  • Or mail it to:
    Illinois State University, Student Health Services, Campus Box 2540, Normal, IL 61790, Attn: Medical Records.

Receiving Your Copies:

Copies of your medical records are available for pickup after 1:00 p.m. the next business day after your consent form has been recieved, Monday–Friday.

  • To pickup your records in person, bring a photo ID to Student Services Building, Room 203
  • Records can also be mailed to the address you provide.

Note: Non-immunization records submitted from outside providers are the property of the original provider and cannot be re-released by SHS. To obtain these, you must contact the original provider. If you have additional questions call patient support at (309) 438-7559 Monday–Friday, 8:15 a.m.–4:15 p.m.
