Benefits

There are two health-related fees included with tuition. The Student Health Insurance fee and the Student Health Services fee support two separate services. Charges resulting from use of the Student Health Services may be covered and can be submitted to any insurance which covers the student, including Student Health Insurance.

  • Coverages and Benefits

    Services for illnesses and injuries are covered. Benefits include:

    • NO annual maximum benefit (previous to 2014-15 policy year, $1 million annual maximum).
    • A "Stop Loss" provision, which limits the student's out-of-pocket expenses for eligible charges to no more than $1,250 per policy year for covered expenses.
    • Policy benefits are payable world-wide and do not require an ISU Health Services referral.
    • $50 deductible per policy year (waived if you have other health insurance*).

    If a student has other health insurance which is primary to Student Health Insurance, the $50 deductible is waived. ISU will consider the balance that is left after the other, primary coverage has made a determination. Student Health Insurance helps pick up family insurance deductibles, copays, coinsurance, out of network charges, and non-covered charges.

  • Benefit Percentages

    • Hospital inpatient and outpatient - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Hospital emergency room (for injury or life threatening illness) - 100% negotiated rate in network, 100% of reasonable and customary expenses out of network
      • Non-emergency illnesses treated in an emergency room are not covered
    • Surgical - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Physician - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Anesthesia - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Dental treatment rendered as a result of injury to sound, natural teeth - 80% negotiated rate in network, 80% reasonable and customary expenses out of network
    • Chiropractic care - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network, limit 12 visits per policy year
    • Mental health - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Laboratory and X-ray - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Ambulance - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Pregnancy - 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
    • Contraceptive Services - 100% negotiated rate in network, 100% of reasonable and customary expenses out of network
    • Preventive Services required by the Affordable Care Act - 100% negotiated rate in network, 100% of reasonable and customary expenses out of network
    • Prescriptions for covered conditions - $50 deductible*, 80% negotiated rate in network, 80% of reasonable and customary expenses out of network
